年擬訂住院病人臨終臨床照顧計劃，將末期
病症長者的服務重心由治療轉為紓緩照顧，
並獲醫院管理局支持，將4A病房設為獨立病
房，提供8張病床用於臨終照顧，讓病人在
私隱度較高及舒適的環境中安詳離世。直至
2017年9月，共有277名病人使用該病房，使
用率超過8成。在東華三院研究基金撥款支持
下，馮堯敬醫院就此計劃進行檢討，結果已在
《亞洲老年學及醫學雜誌》中發表，而4A病
房臨終服務的研究亦已於2017年底完成。
東華三院馮堯敬醫院亦積極參與安老院舍晚期
醫護服務。馮堯敬醫院是港島西醫院聯網的老
人評估組其中一員，與葛量洪醫院紓緩醫學科
攜手合作，為患有末期病症的長者提供到診紓
緩照顧及日間診症服務，讓長者可以在熟悉的
院舍環境和家屬陪伴及照顧下，安詳及有尊嚴
地走到人生終站。醫護人員會與病人及其家屬
商討，在他們同意下制訂適切的預設臨終照顧
計劃，避免接受不必要的救治。此計劃每年為
140名來自20間安老院舍的末期病症長者提供
服務。由2016年4月開始直至2017年9月，共
有206名長者參與此計劃。醫院管理局亦在香
港大學社會工作及社會行政學系的協助下，透
過小組調查，探討預設臨終照顧計劃的成效。
東華三院馮堯敬醫院支援長者離院綜合
服務
東華三院馮堯敬醫院積極推展「支援長者離院
綜合服務」，讓長者離院後有足夠的支援，減
低再次入院的機會。該院為離院長者安排足夠
的病床及老人日間醫院優先應診服務，長者評
估診所亦會為有緊急醫療需要的病人提供服
務。2017年4月至8月，共有140位病人獲轉介
至東華三院馮堯敬醫院。
東華三院馮堯敬醫院加強老人骨科服務
東華三院馮堯敬醫院與麥理浩復康院合作，透
過骨科與老人科協作的醫療模式，為250名長
者骨科病人提供老人科支援，強化港島西醫院
聯網老人骨科的服務，並為髖部骨折的病人提
供全年的物理治療服務。
東華三院馮堯敬醫院冬季流感高峰期
應變計劃
港島西醫院聯網（包括東華三院馮堯敬醫
院）今年再次推行冬季流感高峰期應變計
劃，以減低老人院舍院友到瑪麗醫院求診的
數目。計劃包括加強社區老人評估組的服
務、社區醫護服務及港島西老人院舍的感染
控制，並將社區到訪醫生到訪老人院舍的服
務延伸至週末及公眾假期，安老院舍的職員
亦可於馮堯敬醫院同時為病人一站式繳付診
症費及領取藥物。冬季流感期間，綜合社區
服務組提供了共83次額外的到訪醫生服務，
提供達342次醫生到診服務及3,000次社區護
士服務。服務調查顯示，參與此計劃的老人
院舍均滿意計劃成果，亦樂意再次參與同類
計劃。
TWGHs Fung Yiu King Hospital also actively provided support for End-of-
life services to patients in Residential Care Homes for the Elderly (RCHEs). The
Community Geriatric Assessment Team (CGAT) of the Hong Kong West Cluster
of the Hospital Authority which includes TWGHs Fung Yiu King Hospital,
collaborated with the Palliative Medicine Unit of Grantham Hospital to provide
outreaching palliative care and day-time consultation services to the elderly with
end-stage diseases in RCHEs, offering them an alternative to spend the end of
their life with comfort and dignity in the familiar elderly homes with the company
of their family members. The medical and nursing staff members will discuss the
Advance Care Planning (ACP) with the patients and their family members and
avoid unnecessary clinical procedures. The programme serves 140 elders from 20
RCHEs on an annual basis. From April 2016 to September 2017, 206 elders had
participated in the programme. The Hospital Authority has collaborated with the
Department of Social Work and Social Administration of the University of Hong
Kong to conduct a focus group study to evaluate the effectiveness of the ACP.
Integrated Care and Discharge Support to Elderly Patients of
TWGHs Fung Yiu King Hospital
TWGHs Fung Yiu King Hospital actively promoted the programme of “Integrated
Care and Discharge Support for Elderly Patients” with the aim to provide sufficient
support to discharged elders to minimise the chance of hospital re-admission. The
Hospital provided elderly with sufficient hospital beds and priority appointment at
Geriatric Day Hospital. The Elderly Assessment Clinics of the Hospital also provided
consultation services to patients with urgent medical needs. A total of 140 patients
were referred to TWGHs Fung Yiu King Hospital from April to August 2017.
Strengthening Ortho-geriatric Services of TWGHs Fung Yiu King
Hospital
TWGHs Fung Yiu King Hospital collaborated with MacLehose Medical Rehabilitation
Centre to launch a programme to provide geriatric support to 250 elderly orthopaedic
patients through the ortho-geriatric collaborative care model. The Project aims at
enhancing the ortho-geriatric services of the Hong Kong West Cluster of the Hospital
Authority and provides year-round physiotherapy services to hip fracture patients.
Winter Surge Programme of TWGHs Fung Yiu King Hospital
The Hong Kong West Cluster of the Hospital Authority including TWGHs Fung
Yiu King Hospital implemented the Winter Surge Programme again this year with
the aim to reduce the attendances of residents of Residential Care Homes for the
Elderly (RCHEs) to Queen Mary Hospital. The Programme included enhancement
of services of the community geriatric assessment team, community care services,
infection control coverage to the RCHEs in Hong Kong West District, and extension
of the Community Visiting Medical Officers' service to RCHEs to weekends and
public holidays. In addition, the staff members of RCHEs enjoyed 1-stop services of
settling hospital charges and collecting drugs for residents of RCHEs at TWGHs Fung
Yiu King Hospital. During the winter surge period, the team operated additional 83
sessions of visiting doctor services at RCHEs with over 342 doctors visits and 3,000
community care nurses visits. According to the satisfaction survey, the participating
RCHEs indicated that the programme was satisfactory and they would participate
in similar programmes again in future.
