營辦東華三院水泉澳輔助宿舍
通過公開競投，東華三院獲社會福利署授權營
運位於沙田水泉澳邨的輔助宿舍服務單位，為
期5年，為15歲或以上合資格智障人士或肢體
殘疾人士提供半獨立宿舍生活，並透過提供日
常生活上的協助和督導以及培訓，提升舍友的
生活品質，發揮其潛能。宿舍提供共31個宿
位及1個住宿暫顧，並已於2018年首季正式投
入服務。
營辦東華三院洪福綜合職業復康中心
暨宿舍
通過公開競投，本院獲社會福利署授權營運位
於洪水橋洪福邨的綜合職業復康服務中心暨宿
舍，為期5年。綜合職業復康服務中心有170
個服務名額，為殘疾人士提供一站式綜合職業
復康服務，同時附設50個宿位予有基本自理
能力但因缺乏生活技能而未能在社區獨自生活
的中度智障人士。此外，宿舍亦提供4個住宿
暫顧服務名額。服務單位預計於2018年第2季
投入服務。
小區管理先導計劃
為持續提升院舍服務質素、加強院友的個人
化照顧服務和維持家屬與長者的緊密聯繫，
本院的大埔及北區區域辦事處在2017/2018
年度於東華三院包兆龍護理安老院推展「小區
管理」先導計劃，按認知能力及體能程度把99
名院友區分為體弱、認知障礙、較體健長者等
3個組別，然後安排他們到院內所屬的小區生
活。前線護理員會分組被分配到固定的小區工
作，該院亦按小區院友的需要，制定小區所需
的環境設施配套、照顧服務的流程和院友的
社交康樂活動等。該院餘下的12個房間將於
2018/2019年度全面推行「小區管理」，此模式
亦會逐步擴展到其他院舍。
Operating TWGHs Shui Chuen O Supported Hostel
Through open biddings, TWGHs was awarded the right by the Social Welfare
Department to operate a new supported hostel at Shui Chuen O Estate in Shatin
for a period of 5 years. The Hostel provides services for semi-independent living for
mentally and physically disabled persons who are aged 15 or above. Through the
provision of guidance and supervision on daily life, as well as training, residents’
quality of life can be improved and their potentials can be maximised. The Hostel
has commenced operations in the first quarter of 2018, offering 31 residential
places and 1 residential respite place.
Operating TWGHs Hung Fuk Integrated Vocational
Rehabilitation Centre cum Hostel
Through open biddings, TWGHs was awarded by the Social Welfare Department to
operate a new integrated vocational rehabilitation centre cum hostel in Hung Fuk
Estate, Hung Shui Kiu for a period of 5 years. The Integrated Vocational Rehabilitation
Centre is designed to provide 170 places for persons with disabilities and with
one-stop integrated vocational rehabilitation services, the Hostel also provides 50
places for persons with moderate mental disability who are capable of basic self-
care but lack adequate daily living skills and are unable to live independently in the
community. In addition, the Hostel will provide 4 residential respite places. The new
service unit will commence services in the second quarter of 2018.
Small Group Homes in Purpose Built Home Pilot Project
Tai Po and North District Office of TWGHs started a pilot project “Small Group
Homes in Purpose Built Home” in 2017/2018, with the objective of seeking
continued service improvement, enhancing customised care services and
maintaining residents’ bonds with their families. A total of 99 residents in
TWGHs Pao Siu Loong Care and Attention Home were divided into 3 Small Group
Homes according to the level of cognitive power and physical fitness in order to
deliver customised care services targeting residents living with frailty, residents
suffering from dementia, and residents without mobility concerns. Designated
frontline staff were assigned to the specific Small Group Home to design the
environment, the care routines and the social activities to match the needs of the
residents. The Small Group Home model will be implemented at the remaining
12 rooms of the Home and will be progressively extended to other residential
care homes in 2018/2019.
