Outreach Community Support Services for
Rural Areas
The “Outreach Community Support Services for Rural
Areas” Programme aims to provide preventive and remedial
professional services to support the elderly and their home
caregivers residing in rural villages in Tai Po and North District.
The professional teamwas formed by social workers, doctors,
nurses, pharmacists, physiotherapists, occupational therapists
and professional corporate volunteers with construction and
engineering backgrounds. The team provides professional
services and refers cases to appropriate community service
providers. In the past 2 years, the Programme has extended
its services to help villagers in remote locations in Sha Tau
Kok near the HKSAR border. The team also collaborated
with community partners including members of the District
Rural Committee of New Territories, 3 local social service
organisations in Tai Po and North District and village
representatives, to arrange effective referrals of needy
cases to the Programme, benefitting around 1,800 elders.
「固鄉情」服務
「固鄉情」鄉郊長者支援服務旨在
為居住在大埔及北區偏遠鄉郊的
體弱長者及護老者提供預防性及
補救性的專業支援服務。這支團
隊由專業社工、醫生、護士、藥
劑師、物理治療師、職業治療師
和具備建築及工程背景的企業義
工組成，他們服務有需要的長者，
並按長者的需要把他們轉介至合
適的社區服務機構。過去兩年，
服務範圍已擴展至沙頭角區等鄰
近邊境的村落。此外，「固鄉情」
團隊亦通過與地區的協作伙伴合
作，包括新界鄉事委員會、3間大
埔及北區的社福機構和村代表等
建立合作關係，轉介有需要的長
者接受「固鄉情」服務，總服務人
次約1,800名。
引入創意科技，提升服務效果
Introduction of Innovative Technologies to
Enhance Service Effectiveness
東華三院智能海豹智友趣探訪服務
本院的柏悅長者優質照顧服務站於2017年成立
全港首隊以高科技智能機械海豹為媒介的外展
服務隊，並開設「智能海豹智友趣探訪服務」，
至今已為本院及其他機構的長者地區中心、長
者鄰舍中心、長者日間護理中心及院舍，提供共
65節的小組或單次活動，受惠人數達744人，在
業界大受好評。
三合一追蹤功能防遊走背心
東華三院安老服務部與香港紡織及成衣研發中
心、香港物流及供應鏈管理應用技術研發中心
及香港應用科技研究院合作，為9間長者日間護
理中心及2間專門提供認知障礙症支援的服務
單位購入防遊走背心。此背心配備共3種追蹤功
能，包括無線射頻技術、藍牙通訊及全球定位系
統，保障有遊走風險的長者於室內及室外環境的
安全。在這個計劃下，共有594名長者受惠。
TWGHs PARO Therapeutic Visiting Service
The Group's Prestige Care Services for the Elderly established the PARO Outreach
Service Team in 2017 and has since been offering PARO Therapeutic Visiting Service.
It is the first outreach team in Hong Kong to introduce advanced intelligent toy
seals as the medium of service. The visiting service has reached district elderly
community centres, neighbourhood elderly centres, day care centres for elderly and
elderly homes under TWGHs and other NGOs. A total of 65 group and individual
sessions were provided to 744 people. The Service has been highly welcomed and
complimented by the welfare industry.
Anti-wandering Vests with 3-in-1 Detective Function
The Elderly Services Section of TWGHs worked with The Hong Kong Research Institute
of Textiles and Apparel, Hong Kong R&D Centre for Logistics and Supply Chain
Management Enabling Technologies and Hong Kong Applied Science and Technology
Research Institute to introduce anti-wandering vests for 9 TWGHs Day Care Centres and
2 Specialised Services Units for dementia care. The 3-in-1 detective device features radio-
frequency identification, ibeacon and global positioning system technologies to enhance
the safety of elders at high risk of wandering in both indoor and outdoor environments,
benefitting 594 elders.
