Developing Endless Care Services and

Promoting Life and Death Education

在新界東開展「安辭在家」寧養照顧

服務

承蒙善長慷慨捐輸及董事局撥款,本院的安

辭服務團隊獲得兩筆合共逾2,580萬元的捐

款,開展為期4年的「安辭在家」寧養照顧服

務,把晚期照顧服務推展至家居,為長者和所

有年齡層的末期病患人士及其家屬,提供以

社區照顧為本的晚期照顧服務。服務範圍由

原來的大埔及北區擴展至整

個新界東,由團隊的醫生、護

士、社工及照顧員定期上門

提供照顧。同時,服務團隊亦

聯繫了區內的公立醫院、政

府部門、大學、社會服務機

構、宗教及地區團體等,連結

成緊密的合作夥伴,在新界

東建構一個全面的晚期照顧

網絡,讓居住在安老院舍或

家中的末期病患及其家屬都

能得到全面及專業的照顧,讓

他們安心渡過最後的日子,體

現全人、全家、全程和全隊

「四全照顧」的服務理念。

Commencement of “Farewell Homecare” Project in New

Territories East

Funded by generous donations amounting to over $25.8 million from a kind

donor and Board allocation, the Die-In-Home (DIH) Service team of TWGHs has

kick-started a 4-year project named “Farewell Homecare” in New Territories

East. The community-based project benefits, among others, the elderly as well as

terminally-ill patients of all ages and their family members. The comprehensive

community-based project aims to facilitate a good death for a patient and a

proper farewell with his/her loved

ones. Regular home-based care

services are provided by the medical

practitioners, nurses, social workers

and care workers of the DIH team. The

Team also liaises with collaborative

partners such as public hospitals

in the District, government offices,

universities, NGOs, as well as religious

and community organisations, to

build a stronger DIH care community

network for providing comprehensive

and holistic support for patients and

their family members throughout the

pre- and post-dying process.

東華三院安老服務部參與「樂齡科技

博覽暨高峰會」

東華三院安老服務部於2017年6月16日至18日

參與全港首屆「樂齡科技博覽暨高峰會」,展

示本院有助提升長者生活質素的科技產品及服

務,包括「腦友導航」、「福樂滿心」及「人

生畢業禮」3個手機應用程式、防遊走安全系

統及智能海豹探訪服務等,吸引超過5,000人

次參觀本院的攤位。

為殘疾人士設立虛擬實境訓練

為增加殘疾人士的訓練機會,加強其日常生活

的自理能力,提升治療效果及擴闊生活視野,

本院正積極發展以虛擬實境技術為殘疾人士提

供不同的訓練,包括社區生活技能、體能、認

知及情緒調控等。

Section participated in the first

“Gerontech and Innovation Expo

cum Summit” from 16 to 18 June

2017. During the Event, TWGHs

showcased various IT applications

and services, including the 3

mobile apps of “Cheers”, “Fuk

Lok Mun Sam” and “Funeral

Planner”, as well as the Anti-

wandering System and PARO

Therapeutic Visiting Service, in

response to the elderly’s needs

for quality of life. Over 5,000

participants visited our booth.

Application of Virtual Reality in Training for People

with Disabilities

TWGHs is progressively involved in providing new training opportunities enabled by

virtual reality so as to strengthen the daily functional performance of the disabled,

in terms of community living skills, physical, cognitive and emotion wellness, while

broadening their horizons.

The nurse and social worker in the DIH Service team attentively took care

of an elderly patient with terminal illness, enabling her to complete her life

journey with respect and free of regrets.
