

「關注『腦』朋友」長者精神健康教
育計劃
老年抑鬱症與認知障礙症屬本港最常見的老年
病症。為提高社區人士對長者精神健康的認
識和關注,圓滿人生服務於2017年展開為期
兩年的「關注『腦』朋友」長者精神健康教
育計劃。過去一年,本院安老服務部與香港大
學秀圃老年研究中心合作,進行有關本港長
者孤獨感的調查,調查結果將於2018年5月發
布。此外,計劃透過一系列宣傳短片及活動,
鼓勵社區人士關心身邊長者的精神需要並及早
照顧他們的需要。
「智友醫社同行」計劃
東華三院王澤森長者地區中心及東華三院方樹
泉長者地區中心聯同所屬醫院管理局聯網,在
2017年2月開始參與為期兩年的「智友醫社同
行」試驗計劃,透過跨界別的「醫社合作」模
式,讓患有輕度至中度認知障礙症的長者及其
照顧者,接受相關的護理及訓練,包括認知
能力、機能運作、認知障礙症相關的行為和心
理症狀、身體併發病症、心理社交支援以及護
老者支援等。
「綠色心思」精神健康推廣服務
「綠色心思」精神健康推廣服務以別出心裁的
環境設計和園藝手法,為情緒受困的人士提
供園藝治療小組、綠色體驗工作坊、社區教育
等活動,以推廣正面精神健康的重要性,服務
的對象包括殘疾人士、精神復康者、認知障
礙者、照顧者、青少年、在職人士等。本服
務亦獲社會福利署中西南及離島區福利辦事處
贊助,舉行「藝.無限」精神健康社區推廣計
劃,以改善弱勢群體的情緒和精神健康。

Fostering Mental Well-being in Community and
Promoting Prevention and Early Detection
“Mental Health Healing and Education for the Elderly” Programme
Elderly depression and dementia are 2 of the most common diseases among Hong
Kong’s ageing population. To combat the problems, a 2-year community education
programme “Mental Health Healing and Education for the Elderly” was launched
in 2017. In the past year, the Elderly Services Section of TWGHs carried out a survey
on loneliness among the elderly in Hong Kong in partnership with Sau Po Centre
on Ageing, The University of Hong Kong. The results will be released in May 2018.
Furthermore, through a series of promotion videos and activities, the Programme
aims to promote the message of mental health care for the elderly and to increase
public awareness of early intervention.
“Dementia Community Support Services for the Elderly” Scheme
TWGHs Wilson T.S. Wang District Elderly Community Centre and TWGHs
Fong Shu Chuen District Elderly Community Centre participated in a 2-year
pilot scheme named “Dementia Community Support Services for the Elderly”
from February 2017 with the respective clusters of the Hospital Authority.
The Scheme aims at enhancing support for the elderly with mild to moderate
dementia and their careers in the community through a medical-social
collaboration model. The care plan for individual participants covers the
cognitive power and function, behavioural and psychological symptoms, and
physical co-morbidities of dementia patients as well as the psychosocial and care
support for the elderly.
“I-Greenery” for Social Inclusion and Mental Health Promotion
The “I-Greenery” for Social Inclusion and Mental Health organised thoughtful
gardening and horticultural programmes for people with emotional issues to
promote the importance of positive mind and health. These included horticultural
therapeutic groups, green
experimental workshops, and
community-level promotional
events. Among the beneficiaries
of the Project were people with
physical and mental disabilities,
psychiatric rehabilitants, people
with cognitive disabilities,
caregivers, youths and the
working population. Sponsored
by the Central Western,
Southern and Islands District
Social Welfare Office of
Social Welfare Department,
the “Arts Unlimited Mental
Health Community Promotion
Project” was launched to
enhance the emotional well-
being and mental health of the
underprivileged.
